What percent of graduates are going on to do fellowships after residency?
Is it necessary to do a fellowship with all programs now being 7 years?

The percent going to fellowship is still rather low. Neurosurgery, fortunately, is still a field where it is not a de facto requirement.
As mentioned, many academic positions are seeking a specific niche, with fellowship training. Likewise, many private groups desire that as well.
At the end of the day, it comes down to training and what you are comfortable with as well as what the hospital you work with is willing to credential you to do.
I've looked into it, and there is no one organizations that tracks neurosurgery fellowship placement. There are individual subspeciality societies, so someone would have to compile the data between them.
I do wonder how in-folded fellowships are viewed.
