How do programs look at third year grades? Been getting straight P's in a P/HP/H system so far, but written comments are all good. Other than surgery, IM, and neuro are there specific clerkships that get looked at closely?

Obviously less important than Step I and letters but 35% of successful applicants were AOA (which usually requires close to all H). Like for any specialty, these things matter more at more competitive places.

if you are aoa, no-one looks at grades. They assume you kicked butt in school.
