Either you're trolling or you're really naive. Regardless, the answer is that it's just as competitive (no more, no less) than the other really competitive specialties (eg, ENT, Plastics, Ortho, Derm). All the step 1 scores are likely not significantly different (p > 0.05) between applicants of these specialties. The difference lies in which specialty you did your research in and which physicians you had as mentors and thus, letter-writers. Neurosurgery probably stands out from these because of the demand for quantity of publications. Other than that, most applicants across these specialties could match into the others if they simply had done the research in that field and gotten to know physicians in that field.

Very true. I guess it’s just really daunting looking at the fact that 1 out of 4 applicants don’t match. Especially in an environment where almost everyone applying is more than qualified.
