Getting an academic job

How hard is it to get a position at an academic hospital if you didn’t train at a top 20 program? From what I’ve seen, residents at those programs have access to the resources and people who can make that much easier, it’s a self-propagating machine.

Depends what you mean by an academic position. You can work at an academic hospital as a 100% clinical faculty with little or no research component. In that situation, where you trained is not going to be as big a factor as long as you are competent and can build your practice effectively. If you mean a 50-50 neurosurgeon-scientist position, it will likely be an uphill battle unless you can somehow maintain academic productivity in a residency environment which may not be supportive of that endeavor.
