Do I need a couple of neurosurgery papers during my PhD stage?

Don’t downplay the achievement, own it. Neurosurgeons will respect it highly.

Do neurosurgeons know what Molecular Psychiatry is? If the residency committee sees a first author MP paper (as compared to first author journal of neurosurgery) on the application, would they even recognize it? Or would they just scroll and move on?

The programs you are likely interested in with someone of your scientific background will know what it is - that may not be true at all programs however.

Absolutely you need some neurosurgery papers during your PhD stage. Otherwise they'll assume you became interested during your clinical rotation years. If you are a PhD student interested in neurosurgery, you should be cranking out clinical neurosurgery papers during your PhD years in addition to working on your thesis.
