This is a really stupid question. You do a fellowship to get better exposure to pathologies or truly novice techniques not taught in all residencies e.g. end-vascular or endoscopy. Once you get out into practice you can make your craniotomies as small as you want. Get Teo's book or any of the other 1000 keyhole approach books and save yourself a year. If you want to see the pathologies that a certain surgeon treats, then go watch them for that reason, not for the size of the flap that they turn.
