Still waiting to hear how two graduates 20-30 years ago and an up-and-comer make this an academic "powerhouse". Academics is a game of what have you done for me lately.

Maybe not a "powerhouse" but it is definitely the most academic program in Texas. UTSW has a significantly better culture and operative experience, but Baylor makes up in research productivity and strong connection to MD Anderson. You will graduate a competent surgeon and under Rao's leadership, the program is making more of a splash on the national stage

The bigger "splash" will be when they go back from 4/yr to 3/yr. I don't care ask that much, just can't stand the blatant misinformation. It isn't 2000 anymore, half the programs that get pushed around on here are shadows of their former selves.

My guess would be that Baylor's multiple rotation sites adds volume and diversity but keeps co-residents spread far apart and probably makes for a miserable call schedule. Both programs have new chairs though (Rao and Pouratian) and have historically produced grads who need no fellowship

Not OP, but can speak somewhat to both places based on talking to plenty of students and residents across both institutions. Baylor definitely suffers partly based on what you have brought up. This might be helped by their transition to a 4/year program to help relieve the call burden and residents being spread thin. However, beyond that, I have heard from numerous previous away rotators and home students that the residents at Baylor are not close and there is often animosity between attendings and residents. Nonetheless, there are obvious strengths to Baylor including oncology and critical care along with an established research infrastructure. Plus, culture is so chief resident /chair dependent so it might be better in the coming years under Rao. 

UTSW is the best operative program in Texas. Residents aren't spread as thin and get into the OR heavily even as juniors. As for the culture, it used to be a traditionally malignant program but has shifted significantly over the past 10 years. Residents enjoy hanging out with each other outside of the hospital and have a lot of traditions to help maintain that. The drawbacks are that this program has an essentially nonexistent research infrastructure with a loosely defined research year. However, the hiring of Pouratian is intentional and driven to massively expand the academic side. It will be interesting to see the future of a traditionally operative program with a new research chair.

All in all, these programs are #1 and #2 in Texas depending on what you're looking for in training. Baylor is much more academic and UTSW is much more operative. However, who knows what the future holds for both of these places. You'll be a well trained and positioned surgeon at either.

People need to realize residencies are a lot like sports teams. They’ll have their highs and lows, win some championships and remain at the top for some time but all it takes is some trades (faculty leaving) and change in coaching (new chair) to really redefine a program for better or worse.
