CAST is an accreditation, it means your fellowship met certain standards. Practically speaking, upon graduating from residency some jobs may be looking for candidates with fellowship training and their requirement will likely be that it's CAST approved. This is important considering the fact that any "fellowship" done before your chief year no longer counts as CAST approved, which I believe is a good thing. There's a huge difference in the spine training of someone who spends a PGY-8 year with a Shaffrey or a Uribe, and someone who did an enfolded fellowship their PGY-4 year at their home institution.

So for you this means that when you're interviewing you'll hear all this talk of "residents can do a fellowship in any of these specialties" and "PGY-x is your fellowship year", a good question is "are these CAST approved?" Because if they are not it essentially means nothing. It just means at some point they let you spend more time with an attending of your choice. It won't make you more qualified for an academic job in that specialty. If it is though, and you're talking a PGY-7 fellowship year that's CAST approved, that means you basically cut your residency a year short.

question friends: I thought eras we have until oct 21 to submit (like that is the earliest programs can start seeing apps)

Is this true? Why do some programs say their deadline is Oct 15 then?
