This shouldn't come as a surprise, but if there are any 3rd years following this thread with a Step 1 in the low 230s or worse, please, I implore you to save yourself the hassle and hurt this process will cause you, and look at another field. There is literally nothing you can do to mitigate your score, and you will be screened out continuously. Taking a year off and putting out 5-10 Neurosurgery publications will not make up for it. A PhD with lots of primary author basic neuroscience papers in respectable journals may, but that is not really feasible for someone coming in MD only. You really can't underestimate how much 5-10 questions on that test can really have a profound impact on this cycle and your future career. Thank you, and best of luck to everyone.

I know you are trying to be sarcastic but this statement is actually pretty accurate
This is definitely wrong. I scored in the mid-230s, took a year of, got some pubs, got good letters from strong sub-i's, and now I'm sitting at 15 interviews with a handful of top programs. Not trying to toot my own horn, just trying to point out that there is definitely a proper course of action that can result in good interviews following a suboptimal step 1 score. Once you have the interview, the playing field becomes even so long as your personality and fit for the program can shine during the interview.
