Some programs send out "waitlist" invites (meaning they put you on a waitlist in case an interviewee cancels) later on or a small number of invites in a second wave, but your chances of getting an interview subsequently at most programs is very low.

First of all, I am no expert whatsoever on the subject. However, answering your question, I assume that some programs who have already invited will still have later openings. If you look at the interview dates sheet from the senior society (link on first posts on this thread or google society of neurological surgeons), there are several conflicts in dates when programs are offering interviews. Just to give you one example. One of the only two dates offered by OHSU conflict with one of the only two date offered by both UCLA and UCSF. I am assuming that those who secured the earlier date at OHSU are unlikely to give that one up unless they get interviews that conflict with UPenn and Jackson Memorial. Once UCSF and UCLA send out interviews, people who may have interviews at OHSU and UCSF and/or UCLA may cancel their interviews should they be more interested in those programs. This will also be the case if they take the earlier UPenn and Jackson Memorial interviews, supposing that they are more interested in these programs and that they have been invited to both.
My point is, as the interview season gets more crowded, people will likely start giving up interviews from their less preferred programs. Even if there are no conflicts, those who start getting 20+ invites might not want to unnecessarily increase the financial burden associated with interview season. Hopefully with enough courtesy time for those programs to fill those openings, although every year there are a handful of applicants who either have some last minute changes or are just not thoughtful enough.
I would expect most of the late interview offers to come from mid to low tier programs way more often than from top-tier programs. Although if you look at the senior society sheet, even some top-tier programs may have some dates that conflict one with another.
