What are some reasons that an upper level resident would be fired from a program?

Institutional failure to help a trainee overcome whatever issue precipitated the event.

Firing a senior resident is a SERIOUS red flag for a program. They should be able to figure out whether a resident can cut the mustard during their junior years. If they're firing a pgy5+ it means they have a huge blind spot and don't have a reasonable way of catching problems earlier for remediation.
