There are plenty of smart people at the top schools that it doesn't matter if there are occasional gems at the shit schools. School name is an important factor as it demonstrates a pedigree of excellence (assuming your daddy didn't pay for a wing in the hospital)

It's like professional sports. Take the NBA for instance. The best players are usually selected from Duke. There are occasional undrafted gems but they are. It's better to go with a sure thing from Duke.

The top residencies are going to take the safe bets from those at the top schools. They have to work with this person for seven years. They can't mess around and take a kid from a shit school who might be the next Spetzler. If that kid they missed out on does become the next Spetzler (although Spetzler went to a top school himself), they can just hire him as an attending anyways. They win regardless.

There are plenty of superstars at the top schools that there's no need to take gambles at the shit schools.
