There's still a fellowship, but it's primarily with Tim Smith (a former Laws fellow himself). Tim's an awesome guy, but I'm not sure what kind of endoscopic numbers he does or what the fellowship is really like now.

Is Al Mefty still operating at BWH?

Yeah, he still operates. It sounds like he only does 30-40 cases a year, but no plans to retire any time soon.
