How do residents graduating from good programs end up at bad jobs?

How do graduates from top programs end up at mid/low tier institutions? Is it hard to get a job lined up at the same institution after residency, is that like a 7 year audition of your performance?

Or is there a lot of luck involved as to which positions are available at your home institution/other institutions.

Wow.......just......wow.

One day, you'll realize just how many factors go into selecting a job that will support you and your family. And you'll realize how far down institutional prestige is on that list.

Seems you're early in your neurosurgical career. Focus on doing a good job for you and your patients, and learning as much as you can. The rest will come.

If this sounds dismissive, it's meant to. You need to squash this line of thinking ASAP.

This is an unanswerable question. What kind of job? Subspecialty? Which hospital at your "institution"? What's the job market like? What case mix do you want, and what do they need?

Why even ask this? Are you trying to match at a place where you'd like a job? I think this is setting yourself up for disappointment.  You're never guaranteed to match at that institution, and even if you do, you're trying to predict a job market 7+ years out? This is a silly way of thinking about it.
