Loma Linda has had issues with ACGME. The have had a few ex-residents go through the pre-residency fellowship without much positive results. Better and more known programs are the BUMC and Miami pre-residency fellowships.

The goal in these programs are to be clinically competent, well liked, and to get a LOR from well known figures in neurosurgery who can back you up when an opening arises. Don't get tricked into becoming scut labor for a program knowing that the your planned outcome isn't likely. Follow track records of previous applicants. At the end of the day it is "playing the game", unfortunately politics and likeability have a higher yield than your actual knowledge. Most medical school graduates and NSG applicants have the work ethic and knowledge, but it is the social factors that limit people.

I would recommend going to another categorical speciality and go to grand rounds, do research, etc. This way when a neurosurgery opening arises you come from a positive pool. Also, in the worst case, you end up board eligible in something else.
