Many top programs do not like to go down their rank list at all, and so will match the top number of applicants for slots they have. If you were in consideration for a rank to match position, by not declaring the program as #1, you may count yourself out for a position. I don’t know why this is so important to Chairs and PDs, but it is a reality nonetheless.

Agreed. The gamble with BNI is that they don't tell you if you are ranked highly (unlike many other programs) so if you use your #1 card on them and the feeling isn't mutual then you might drop down another program's list where you would have matched if you had sent them that email... subsequently dropping down your own rank list

This is something weird about neurosurgery that isn't always appreciated - the #1 email/mentor phone calls do have sway at a decent portion of places, so using it on BNI where like 10-12 are trying to get in is a gamble. If you used that card on maybe a less "sexy" program that you'd really be almost as happy at then they may be more likely to rank you to match, vs. if you do it at BNI and put that other program #2 and then don't match Barrow, you're more likely to fall past that other program too.

In the end it's likely best to still rank according to preference, but it's a consideration that isn't accounted for by the "rank only as you prefer" philosophy, if you're thinking about ranking one of these really desirable programs first

As someone who matched last year this is one of the truest posts on this site. Ranked BNI #1 and as a result matched at my 13th choice even though if I had sent my #1 email to my 2nd, 3rd, or 4th choices I 100% would have matched at any of them. #2-4 all had told me they wanted me, but when they realized I wasn’t ranking them #1, they looked to other applicants that wanted them more than me. Don’t fall for the lie that “the rank system favors applicants so rank your most fantastical dream #1, it can’t hurt you.” This is so very false.

Depends on your mentor I suppose. If they truly are invested in your future then they should be willing to receive your honesty without retaliation. Just be honest tell them that you appreciate everything the program is done for you but for XYZ reason you would appreciate if you would call blank program to support you.
