Hypothetical question: If you are fellowship-trained in a subspecialty and entering an academic practice, is it possible to not do any spine even as a junior attending? 

Nothing against the field, just not my cup of tea...

Yes of course. If you are being paid on a RVU basis you will take a pay hit, but your colleagues will like you for the extra spine they get. If you have a set salary then your MBA or PhD or RN (nowadays) manager might want to "talk" about your productivity.
