It is usually 1,000 cases per year for a 1-resident-per-year program. Cases also have to be varied and meet ACGME and RRC requirements. If a program does 1,500 cases it will have a 2-1-2-1 structure and be approved for 11 residents total - if they increase to 2,000 cases per year, then they will be approved to have 2 residents per year and get complement of 3 residents (maximum of 14 residents in the program).
How/when to allocate is up to the program. Most will do it over time so they consistently have 2 residents per year. However, if they wanted to they could hire an extra 3 interns at once, or hire 2 extra interns and 1 PGY3, etc. But there can only be 14 residents in the program at once. Now, these numbers might have changed, and as I said, it is not about the raw # but also the type of cases. Your P.D. will have more updated info.

How do you talk a program into backfilling a spot when they are under compliment- about to be a pgy3 and attempting to transfer but every program says "we aren't alloed to backfill"

You can’t talk them into it…it’s a long process to increase complement and justify their operative volume to ACGME. Were you expecting these programs to start the process because you decided to Call them? Lol.

Rules have tightened about it. With complement increases the expectation going forward is that you have to fill with PGY1s in the match.

Was super lame for Emory to overfill, resulting in two intern spots this year
They interviewed 60 people for those two spots, and was a big fat red flag for when I made my rank list

My advice would be that life gets better as you get more senior regardless of program. If you are truly utterly unhappy then I would suggest you go talk to a mentor or your program director.
