Spine and functional sounds like a good move. Have seen many examples successfully hired. Likely because they can cover their own salary most of the time. Probably comes with a nice pay raise as well.

No one in PP is gonna give out a pay raise just cuz you did a functional fellowship. Why would they pay more for a field no one cares about, that doesn’t bill well, and that realistically will only divert a surgeons attention away from what matter (spine). The only people who care about functional are other functional nerds.
If you do a spine or vascular fellowship, then yes they will pay you more, because those are high billing procedures (or they’ll be glad to have you in a call pool). Functional guys are fairly useless.

If you hate the idea of having a significant other or a family, then you will be paid better for taking stroke call on top of neurosurgery call. Otherwise, the only purpose of a fellowship from a private practice perspective is to generate a wider referral base.
