Advice as I progress into the second half of medical school on my chances?

Step1- 239
Research: 19 publications, 8 first authors. All sports medicine, cancer related. 2 other first authors under review. 5 conference abstracts. No neurosurgery specific research. Also have a drug delivery patent. 

Was averaging 255 on practice tests and was not expecting this outcome. Nothing to do but move on from here. Is step1 a deal breaker? What do i need to focus on going forward?

Step isn't a deal breaker especially with those pubs. Try to at least write a neurosurgery case report this year just so you have one under your belt.

How are your grades? Do you go to a fancy med school?

Top 40 Med school, Midwest, top 3rd of my class. We employ a H, HP, P, LP system. Most grades are H/HP.
