How do your programs decide who gets their requested vacation? Seniority or first come first serve?

typically seniority. We manage it ourselves at my program.

I think they understand the concept, just laughing at the naivety that it would be first come first serve

At my program, the Chief makes the schedule including residents. Requests are granted based on seniority. Seniors will get bookend weekends off, juniors just get one weekend off. Only chiefs even have the option of taking more than one week off at a time, and its only happened once.

Do residents generally take all of their allowed vacation in a year, or is there pressure to minimize vacation?

Pressure? WTF.. It is against ACGME and for sure your institution's regulations to be pressured not to take the full vacation time agreed upon in your contract. What part of the country is this in? Midwest here, never seen this happen.
