No, I believe it is an individualized decision. Different programs are willing to overlook absence of step 2 IF they like the rest of your app. If there is another weakness (e.g. poor step 1 or weak research), many will put you in tier 2 pile where you get some interviews, but you also end up in limbo land (school sends out some interviews and some rejections; you receive neither) for many places.

As you can see, nobody cares about Step 2 CK that much in neurosurgery...

Perhaps not for interviews buddy. But they do care for if when making their rank lists. My home program won't rank you unless you have it. MGH invited today and they also require step 1, 2 CK, and 2 CS for making rank lists.

there are also several others that do fit this bill.

My apologies, you're right, I meant in the context of getting interview invites. My understanding is that most programs want it for rank list so they know you didn't fail the exam. Also, I'm sure it varies from program to program but I know some actually care about how well you score, while others simply want to know you passed the exam. The point I was trying to make was simply that it is not important to have to obtain invites.
