I am interested to hear about the different call systems especially for small programs 1 or 1-2-1 per year...Is it typical for PGY4 and 5 to take 7 calls per month? How other programs protect their senior residents from primary calls? Night float system? It really sucks to take frequent primary calls during senior years.
Our program is small and uses the 2-4 maximally with the 5 to fill in the gaps. It’s hard to outsource some call to midlevels we found out because it’ll never fly to claim call is scut no matter how many calls you have done, and there’s no way to make it make sense in the budget when you have us as indentured laborers for free, unless the midlevels also steal the procedures when “no qualified resident is available to assist.”
We are 1/year. Our call:
PGY2: 10/month
PGY3: 8/month
PGY4: 6/month
PGY5: 4/month
PGY6: 2/month
1/year in a spot with a bunch of midlevels.
Pgy-1 takes buddy call with the pgy-3: 6/month
Pgy-2 takes 6/month
PAs take the other 18 nights.
Pgy-4/6 - home call for OR cases only
Pgy 5 - protected research
Pgy 7 - chief call
We're a new program so that helps.

The big problem with small call pools like this is not so much the written down call but what happens if even a single resident goes dark for significant amount of time. The reality is 7 years is a very long time in life happens to a lot of us in ways that we don't anticipate going in. Having a small call pool like this means that even one resident going down could result in a massive amount more work for the others. That is why for small programs is very important to investigate exactly what forms of ancillary support are available.
Any other programs out there that don't get the post call day off?
