Residency Rivalries

Are there a lot of rivalries between programs in cities/states? How does this manifest?

MGH and Brigham
Cleveland Clinic and Case Western
Penn and Jefferson
Baylor and UT Houston
Rush and Northwestern
Maryland and Hopkins
The NYC programs
Pitt and Allegheny
Cedars-Sinai and UCLA

Usually one is slightly more academic than the other.

This is not accurate at all, look at the profiles of the residents, they are all very dissimilar from each other. Nobody was deciding or even interviewed at these pairs of programs.
