Stay away from the big name labs like Aghi and Brem. Look for a place where you won't have to compete with too many others for a primary authorship so you can justify taking a research year.

I haven't taken a research year but places where I know FMGs and US grads alike have had success:
1) Pitt: obvious clinical powerhouse
2) Northwestern: up and coming labs with ridiculous funding.
3) Brigham: good mix of clinical and basic science. Pituitary and skull base publications galore.
4) OHSU: good if you're thinking about functional/epilepsy. Faculty are pretty welcoming from what I've gathered.
5) Florida: good amount of cerebrovascular and functional papers coming out of here
