Something similar to this was discussed not too long ago. Lots of disagreement on which programs are ranked where but maybe will give you some general ideas.

I think Doximity does a good job answering question 2 and 3. Usually number of attendings and residents can give you a feel for patient volume. How much residents actually operate is difficult to interpret as well. Some programs will tell you number of cases logged for residents if you ask for it. Programs like UCSF, Barrow, Pitt, NYU, Jefferson, USC, and UWash all have great case volume and operative experience.
