But they recently hired some young faculty and are transitioning to Welch as chair though so the future is bright.

Doximity rankings are essentially based on a survey of neurosurgeons on doximity consider to be the top 5 programs. This then gets summed up. Hence, if a program has more faculty members on doximity, they are likely to rank their own programs highly and this can result in very biased outcomes. Ranking based on research output are slightly more reliable, similar to USNWR research rankings for medical schools.

I personally consider programs on a binary scale as either established or not. Most places will give you good enough operative and academic training but if you want an academic job then it would behoove you to go to as brand-name of a place as possible.
