Besides the obvious personality issues, what are some reasons applicants don’t match especially if they go on ~15 interviews?

What quality improvement projects have your performed at your hospital and what was the result?
It would be helpful to create a list of successfully implemented ideas for residents at other institutions to implement over overall improvement of the quality of care we deliver to our patients.

| Spending research years at a closeby institution? |

Is this generally possible? If you are in UCSF, could you arrange to work under a PI at Stanford (assuming you had funding) during your two research years? Other examples: UCLA resident at USC or Caltech, Baylor resident at Rice or Houston resident at Baylor, Columbia Resident at Cornell etcetc.
If there is no already-present consortium between two institutions, is this possible? Perhaps might be easier if you are working for two PIs - one at home institution and another at the other uni.

| UVA vs UVA-NIH |

Besides spending 3.5 years at the NIH vs 2 years of research in the UVA program (which apparently can also be done at the NIH), are there any big differences between the two? I'm guessing both set you up well for academic neurosurgery.
Does anyone know what the clinical experience is like at the NIH?
