I am starting an MD/PhD program soon and neurosurgery is of potential interest. Career goal is physician scientist with 60%+ protected time. I am concerned that the long period away from the lab during residency will make it difficult to catch up once research years resume. Are neurosurgeons at a significant disadvantage for basic science because of this?
In general, are neurosurgeon-PhDs not as well positioned for basic science careers compared to neurologists?

If you have an interest in a single subspecialty, what is the maximum proportion of cases that you can do in that area? Like, can you do 90%+ endovascular cases and have the lower residents pick up the rest? Or will that get you labelled as an asshole (and somehow negatively affect you later down the line)?
What are the boundaries for your freedom as Chief resident? (in terms of delegate case load/type for yourself?)
For example, program website description of chief year are a bit confusing:
"The chief resident is given preference for selection of operative cases. The chief resident’s clinical experience involves performing most of the complex surgeries on the neurosurgical service, including: ..."
Does this mean that for whatever area you prefer you do the most intensive cases/have the most responsibility, or does this mean that you select some proportion of your cases but the rest of your workload is reserved for highly intensive cases?

Besides the SANS modules, please let me know what other resources needed to comfortably pass the boards and how far from march to start studying. I haven’t passed last year and would be in trouble if I don’t pass this time.

Were your future spouses reluctant with the idea of you pursuing Nsgy residency? Assuming you just got married during 4th year, that would put you in residency right at the start of your marriage. I cant imagine that all new marriages can survive that. Any advice for determining whether your future spouse would be up for basically waiting for you for seven years? Red flags that indicate a weak marriage?
