  Best book on spine surgery

Can anyone recommend a good book for spine surgery for residency and beyond? I’m looking for a book that isn’t too inundated with angles and biomechanics and can give practical evidence-based advice on when to consider decompression v. disc replacement v. fusion etc. I know much of what we do in spine surgery is dealer’s choice but there are certainly intelligent arguments to be made for surgery A v. surgery B v. no surgery.

Question Can long term untreated AAI ultimately cause functional LLD?

Patient diagnosed with cervical transverse ligament rupture with subluxation at c1-2 from traction injury. Odontoid peg intact, no dens fracture or cord impingement apparent. Skull held at oblique angle for over two decades until pronounced ridge of muscle observed to one side of spine, lumbar and knee pain with functionally short right leg observed from pelvic shift. 
Skull spontaneously fused to c1.
Surgical fusion posited on initial diagnosis but never pursued. 
Would stabilisation help at this late stage? Patient is in fifth decade of life.

  Working out as resident

Incoming intern here, do y’all get time to lift? Currently lifting 2hrs/day, 6x/week and worried my current program is overly ambitious for what’s to come. Energy levels aren’t the issue as I’m basically snorting pre- at this point, but moreso having time after work to spend an hour or so at the gym without compromising on sleep/studying too much. If any of y’all have made it work, and continued to make progress during the worst parts of your residency, I’d love any tips you have. 

Also, wtf do y’all eat outside of cafeteria food?

  Cornell vs Hopkins vs UCSF medical school

My friend was awarded a full cost of attendance scholarship to Cornell and Hopkins and full tuition to UCSF this past cycle. In the end they ended up choosing Cornell as her support network is in NYC and they wanted to stay in New York. They absolutely despised Baltimore and given that they had the option of staying in NY, they decided against moving across the country. 

Given their interest in NSGY, they are now seriously kicking themselves over their decision (especially turning down Hopkins). How do I help them understand that this wasn't a mistake (unless people on here think it was)?

  Elective spine only jobs

Anyone have thoughts on these elective spine only jobs? Often tied in with like an ortho or combined ortho/neuro spine group. Especially as a first job out (if really all you want to do is elective spine).

It could be a great fit for me but I’m worried if 1) it will impact board certification cases (will they actually board you in neurosurgery if you didn’t do a single crani for the oral boards) and 2) if you pinch yourself down to that alone for the first couple years would that limit me to those types of jobs and not general neurosurgery community jobs in the future should I need to relocate or something?
