Sorry I was not clear. I meant do I send the programs I will rank 2-5...so very highly but not number...to let them know I will rank them very highly...without lying obviously
Along the same lines...do people typically say "I will be ranking you #2" or just "will rank highly"?

If the residents (often PGY5s) have a very active role in the process, then you should absolutely let those residents know (if that is your honest intention).

No resident, junior or senior attending effectively knows if his/her vote counts, unless you are the one entering the results. There are last minute changes, no matter how open the process may be. I am sure there are more or less democratic programs but ultimately it is a prerogative of the chair/pd combo.
No chair or pd will ever take a resident just because the rest of the faculty, or residents, liked or voted for them.

Yes in the end only one person submits the list, but a good chair/pd will at the very least NOT take someone that the rest of the faculty/residents hate.

Like everything in the match, it's pretty program dependent. Some places call a significant proportion of their list, while others limit the number of calls to those ranked within the number of positions they're offering. It's a spectrum so it's hard to put too much stock into it. Advice I've always gotten was if they're already a reasonably highly ranked program for you then you might consider bumping them a bit, otherwise don't do too much with it.
