Thanks to everyone who has helped so far with the Wiki! I'll try to help fill in things as I can but I have been rather busy lately with other things.
The great part about a Wiki is it is community sourced and driven. That means you guys play a major part in what's there both by editing and putting things you want, or letting others know what you'd request so it can be created.

Hi all,
I am currently an MS2 at a mid-tier MD program with an okay NSG residency program. I am interested in pursuing a career in academic medicine and would ultimately like to have a shot at a program at a respected academic program like Barrow,UCLA,Pitt, UCSF, etc. I have one first author publication on a case-report in a good neuroscience journal, and I have two other clinical pubs in the works from this past summer. My grades put me in the top quartile of the class, but I should be top 10% and AOA eligible by MS3. Besides striving for AOA and a 250+, what other advise could you give me?

Can we discuss this? UC Irvine charimanship has been a revolving door for the last 10 years. The Davis chairman got canned for infecting GBM patients. UCSD abandoned ship for greener (ivy) pastures. Leaving UCSF, who is also in a quiet chairman search and lost their cerebrovascular service to a private practice. What is going on in California?

How do you guys keep your case logs? I've seen a variety of ways. Obviously, the ACGME log is necessary. How do you decide which level to put (assistant, senior resident, lead resident)? For a case that is booked as one case but has multiple parts (e.g. back/front spine, xlif + perc screws, crani for aneurysm w/ intraop angio) do you log that as one? Do you keep anything else as a log? I've seen moleskin notebooks with patient stickers or info. I've seen binders of op notes. One guy had a searchable database with pdfs.

I'll get this started with an interesting one. It's been a while, so I don't recall the specifics but here's the gist:
Patient was on the medicine service and had a pulmonary empyema. IR was consulted and placed a pigtail catheter. We get called days later for paraparesis. The patient can't get an MRI, but ct-myelogram reveals the pigtail is somehow entering through a throracic foramen and traverses the spinal cord.
